Register your student athlete by creating a family account. Then add student. You will need to upload the following documents during the registration process:

• Medical Physical *Completed May 1, 2025 or later
• Proof of Insurance
• Activity Code of Conduct
• LCSD1 Liability Waiver
• Walk Home Waiver (optional)
• Students who are home schooled or attend LCDS1 virtual school and live in the Johnson attendance area may participate in Johnson's Athletic Programs.
